Low Secondary Boost -- How did you fix it?
 
I know that plenty of FD owners have encountered low to no boost from the second turbo. And I've read the troubleshooting guides that are really quite well done.

What I am wondering though is Exactly what you found/fixed to correct a low boost from the Secondary.

Really interested in getting a feel for the frequency of the different problems that cause the boost issue.

Umm typically you need to rebuild or replace your secondary turbo. especially if it was boosting fine but over time dropped.

I real good sign of needing a rebuild, due to a bad secondary, is if it his full boost after transition but drops off more and more as the rpms rise.

Check bypass valve on secondary side, it could be blowing open.

If engine has been blown, the bits could have ruined the turbine wheel in the 2nd turbo.

It could be a leaky diaphram in the switching actuator in the exhaust manifold or leaks in the vacume lines going to them.

It could be bad bearings in the turbocharger as Chris stated.
